4. Direct Primary Care Model & Insurance Disclaimer
Lotus DPC operates under a Direct Primary Care (DPC) model.
We do not bill insurance for membership fees or included services
You may submit receipts to insurers or use HSA/FSA funds where permitted, but reimbursement is not guaranteed
Medicare/Medicaid participants remain responsible for compliance with program rules
Membership details are governed by your membership agreement.
5. Patient Portal & Telemedicine
Patient portals and telemedicine services are subject to additional terms and consents.
Telemedicine is provided only where legally permitted and when clinicians are appropriately licensed. You must be physically located in an eligible jurisdiction at the time of service.
Electronic communications involve inherent risks. We implement reasonable safeguards but cannot guarantee uninterrupted or error-free access.
6. Memberships, Payments, Cancellations & Refunds
Membership fees, benefits, billing cycles, cancellation terms, and refunds are governed by your membership agreement.
Unless otherwise required by law:
Setup/enrollment fees are non-refundable
Monthly plans are not prorated
Prepaid plans may be eligible for prorated refunds for unused full months, less discounts and fees
Outstanding balances remain due upon cancellation.
7. Scheduling, No-Shows & Late Cancellations
Appointment availability is not guaranteed.
No-shows or late cancellations may result in fees as disclosed on the Site or in your agreement. Repeated no-shows may restrict scheduling access.
8. SMS Communications & Opt-Out Instructions
By providing your mobile phone number, you consent to receive SMS text messages from Lotus Direct Primary Care PLLC related to appointment scheduling, membership information, billing notices, and other service-related communications.
Message frequency may vary. Message and data rates may apply.
You may opt out of receiving SMS messages at any time by replying STOP to any message.
After opting out, you will no longer receive SMS messages unless you re-enroll.
